We're so excited you've chosen It Takes A Village! Below you'll find all the forms required by the State of Texas for enrollment in a registered childcare home. Please download, complete, and return them to us.
Admission Agreement
The primary enrollment contract between you and It Takes A Village. Covers care schedule, fees, and responsibilities.
Child's Emergency Information
Emergency contacts, authorized pick-up persons, and medical information for your child's safety.
Health Statement
A record of your child's health history, allergies, dietary needs, and current medications.
Immunization Records
Up-to-date vaccination records are required by Texas law for all children in registered childcare homes.
Media & Photo Release
Optional consent for us to share photos and videos of your child in our daily updates and social media.
Transportation Authorization
Required if you'd like your child to participate in field trips and off-site activities.

Welcome to the Village!
Submit your $100 deposit (applied to your first month) and your child's spot is secured. We'll schedule a 1-day trial to make sure it's a great fit!

Payment accepted via Venmo, Cash App, or PayPal. Bi-weekly payment option available ($500 for full-time).
